Housing assistance
Will assist appropriate individuals to obtain section 8 housing, or low cost rental properties or assist appropriate individuals to obtain appropriate mortgages for first time housing.
Auxiliary services assistance
Each of the regional community collaborative will partner with utilities, internet and cellular providers, and federal, state and local authorities to provide appropriate applicants with receiving or supplementing existing services, to assure continuity and stability within the home, providing a more stress free environment as well as the necessary tools to become contributing members of the community...
Home care services
Create Partnership with a regional home health agency to provide home health aids, home nursing, hospice care to appropriate applicants to help keep the family u it together, showing continuous respect to older or infirmed family member, providing a more familiar, stable environment affected family member while providing the need support for the family to have the family member at home with the least amount of stress and emotional distress.
Healthcare services
Create partnership with a local/regional health care provider to develop an on-site medical office providing internal medicine, family medicine, pediatrics, gynecology, obstetrics, dental and vision care. It would be the goal to also partner with a local pharmacy to provide prescription medications. The regional collaborative would also apply to the bureau of primary health care in Washington DC for a federally qualified health center designation to allow monies to be obtained to provide health care to the un and under insured, as well as, to receive the enhanced Medicaid fees for services rendered to Medicaid clients.
Food services
The regional collaborative will partner with a local/regional pantry or food distributionservice, to develop an on-site pantry for appropriate applicants to get groceries to prepare home meals. The regional collaborative will also create a food service to prepare three meals daily to be served in the open collaborative dinning room and also to be delivered to such and clients.
Battered women and family services
The regional collaborative will partner with local/regional battered women organizations to provide safe shelter, counseling l, mental health, addiction counseling and child card to appropriate applicants. The regional collaborative will develop a 25 bed simple woman unit, as well as, 5 four person family units; providing sleeping areas, bathrooms and showers, and common areas l for recreation and rehabilitation. The women and child would be fed through the collaborative dining room, receive medical care at our collaborative health care facility, be assisted through social services and have mental health and addiction services also available to them. Each woman will receive a cash stipend to help find a secure home and can utilize housing assistance program of the collaborative. Additionally the women with children can utilize child care services while seeking employment.
